The state of Illinois requires a full health examination for students entering Pre K, Kindergarten, Sixth and Ninth grades, and for any student entering an Illinois school for the first time.  All health information  required for school entry must be submitted prior to the first day of school attendance.  Students who do not have a completed  physical exam form dated within one year of the first attendance day will be excluded from school until a physical exam is submitted.
